About our Transition service
This service focuses on supporting young adults with learning disabilities between the ages of 16-25 looking to move from their parental home into supported living. Our support workers ensure all the people we support gain the skills and knowledge to live as independently as possible and support them throughout this transition. The service provides 3 stages of transition from supporting people within their current home, to moving in to our Hft service and the final phase of moving out into a supported living service. You will be welcomed in to the team and coached along the way and will also receive outstanding training for your role.

What will you be doing?
- Giving the highest possible standards of support to individuals with moderate to severe learning disabilities.
- Supporting them to discover new interests, reaching goals like travelling and holidays and working positively with their families and friends.
- Helping individuals to engage in different leisure, home or work-related activities, all according to their personal choice.
- As a Support Worker (Care Assistant), your responsibilities are varied and no two days are the same.
- As well as providing physical and emotional support you may be needed to help with household tasks and personal care as well as supporting individuals with healthcare needs, e.g. taking medication.
- Everything you do will enable people to have choices about their own lives, providing flexible services that meet people’s needs, and always looking for the best way of doing things.

The Service is located close to Biggleswade town centre, a driving Licence is not required as long as you live in or around Biggleswade or have the ability to get to the service. Full and part time opportunities available. You will need to have flexibility to work shifts, including sleep in nights, weekends and bank holidays.

- Sleep ins at the service are @ £45.06
